Because the N461 form is submitted separately and not in one PDF with your bundle, despite the Admin Court rules saying a Litigant in Person merely has to explain why they cannot comply with the minutiae of every diktat.
These are not HMCTS employees making mistakes!
They are the very gatekeepers of the swamp who are tasked with dissuading you from ever getting your case into court by making you ascribe their mendacious actions as mere mistakes instead of their core purpose.
Martin, that is a very generous approach to settling all this mess you are in - "let's all just admit we messed up, fix the mess and move on." I hope it works.
Interestingly, if your olive branch is ignored or even rejected, though, we will have to return to the idea that all of this mess is in fact deliberate. A deliberate incursion into the Law domain by the Executive/Administration domain, which has no place in the Law domain but is there for, what, simple convenience for removing money from citizen wallets? Or more nefariously, using administrative double-talk (including false metonyms) to confuse, control and enslave the citizens so subtly they never realize what's going on? The first is deplorable, even criminal or unconstitutional, but the second - call it by its real metonym - is outright Evil.
Let's hope those who are performing this mess see the Light. Fixing only the metonyms will not be sufficient. They also will need to reset their hearts. Nothing less will truly fix this mess you have uncovered.
